A case of Brucella septicemia presenting at the onset as a severe micr
oangiopathic hemolytic anemia with coexisting dramatic hemorrhagic syn
drome (severe epistaxis, gross hematuria, and skin purpura) is reporte
d. A hemogram showed severe thrombocytopenia, anemia, and leukopenia.
Bone marrow morphology showed the typical features associated with Bru
cella infection: numerous histiocytes with signs of activation, multip
le granulomata, giant cells, and hemophagocytosis. After appropriate a
ntimicrobial therapy, the clinical and hematological status of the pat
ient improved, and he is alive and well 1 year later with disappearanc
e of all hematological abnormalities.
