Though cystic hepatic lesions may have features that help characterize
them by noninvasive imaging modalities, these findings are often nons
pecific and may be unable to establish the presence of malignancy. Thi
s is illustrated here by describing an epidermoid cyst of the liver co
ntaining microscopic foci of squamous cell carcinoma. The importance o
f totally resecting an epidermoid cyst of the liver is emphasized.
