A 26-year-old Japanese woman, gravida 2, para 1, was referred to our u
ltrasonography clinic at 27 weeks' gestation, because of a suspected f
etal heart anomaly. The hemodynamic examination, specific for hypoplas
tic left heart syndrome and obtained with color Doppler sonography, ga
ve a clue to an accurate diagnosis of this entity. Moreover, the use o
f magnetic resonance imaging provided information complementary to the
sonographic findings. The sophisticated type of fetal cardiac examina
tion with color Doppler sonography and magnetic resonance imaging was
very effective to interpret hypoplastic left heart syndrome with total
anomalous pulmonary venous return. An accurate prenatal diagnosis of
congenital heart anomalies is useful for obstetrical management.
