A promintent U wave induced by exercise testing in patients with anter
ior wall myocardial infarction is a highly specific but insensitive ma
rker for the narrowing of the left circumflex or right coronary artery
, although a U-wave increase alone with or without ST-segment depressi
on may help better identify these coronary artery narrowings. In patie
nts with inferior and/or posterior myocardial infarction, a prominent
U wave is regarded as a simple electrocardiographic marker for inferio
r and/or posterior residual ischemia.
