A pertinent leukaemia with refractory anemia excess blasts in transfor
mations terminating as Ph negative chronic myeloid (CML) is described.
It is believed that the initial picuture of RABBET as well as the ter
minating picture of CML was caused by the same abnormal clone. Myelody
splastic syndromes are among the most frequent haematological malignan
cies, particularly in the elderly. The MSD are characterized by ineffe
ctive hematopoiesis peripheral blood cytopenias and hypercellular or a
narchic bone marrow. There are qualitative and quantitative abnormalit
ies affecting all cell lines. (1) According to Fab classification, the
re are five types of MSD which include: 1) refractory anemia (RA), 2)
RA with ringed sideroblasts (RARS), 3) Refractory anemia with excess o
f blasts (RAEB), 4) RAEB in transformation (RAEBt) and 5) Chronic Myel
omonocytic Leukaemia (CMML). Refractory anaemia is probably the common
est subtype of MDS(S). Median survival for patients with MDS ranges fr
om four years for RAS to three year for RAEBt. We describe a patient w
ith RAEBt Type of MDS who developed picture like CML after a period of
two years.
