A case is presented of a 45-year-old woman who developed parkinsonism,
cerebellar dysfunction, aphasia and flaccid paralysis secondary to he
at stroke. Magnetic resonance imaging revealed irregular patchy areas
of increased signal intensity in the white matter of both cerebral hem
ispheres and the left striatum, remaining unchanged over a follow-up p
eriod of two years. Cerebellar atrophy, while not seen on initial exam
ination, was present on the follow-up study done one year later and be
came more marked after two years.
