The substrate of the permanent form of junctional reciprocating tachyc
ardia is an accessory pathway with no spontaneous anterograde conducti
on, usually located in the posteroseptal area, We report a case of thi
s type of tachycardia with overt anterograde ventricular preexcitation
. Electrophysiologic study confirmed that tachycardia was due to an ac
cessory pathway with long retrograde conduction time; electrophysiolog
ic findings suggested longitudinal dissociation of the accessory pathw
ay, Radiofrequency application at the coronary sinus os resulted in di
sappearance of preexcitation and cure of the tachycardia.
