When the pathomechanism of erectile dysfunction is obscure, intraureth
ral administration of prostaglandin E1 is followed by a marked increas
e in blood flow velocity in the penile arteries as measured by color-c
oded Duplex sonography. This increase compares to that following intra
cavernous administration of half the dose of prostaglandin E1. The dif
ference in the effects after intraurethral and intracavernous administ
ration is the missing rigidity after intraurethral application; there
must be a loss of pharmacological efficacy to the smooth penile muscle
s. Clinical efficacy is to be expected with higher intraurethral dosag
e.
