A case of massive, anterior, chest-wall reconstruction is described, u
tilizing an extended TRAM flap design which was microsurgically transf
erred on both deep inferior epigastric pedicles, in order to perfuse t
he flap maximally. The flap is capable of reconstructing the largest c
hest-wall wounds and should be added to the armamentarium of those rec
onstructive surgeons involved in the care of these patients.
