A preterm male infant born at 33 weeks of gestation developed respirat
ory depression and apnea at similar to 20 h after birth. Laboratory te
sts indicated severe hypermagnesemia, acidosis, and hypercalcemia. Cor
d blood and maternal blood concentrations of magnesium were normal. Th
e effects and possible causes of hypermagnesemia are reviewed. The inf
ant recovered with treatment, although the etiology of his hypermagnes
emia remains unknown.
