We describe a case of pulmonary hyperplasia associated with tracheal a
tresia and a complete obstruction to the egress of pulmonary secretion
s. In classical pulmonary hyperplasia associated with cartilagenous la
ryngeal atresia and a persistent pharyngotracheal duct, the histologic
appearance of the lungs is normal but exhibits ''synchronous'' hyperm
aturity. The histologic patten in our case is much less mature, resemb
les CAM type III, and exhibits ''asynchronous'' development. We sugges
t that these histologic patterns be distinguished and that pulmonary h
yperplasia is probably underrecognized and not nearly as rare as previ
ously thought.
