Patients with anterior knee pain and lateral retinacular tightness usu
ally improve with nonoperative management but occasionally require sur
gery. Patients who benefit most from lateral release have pathologic l
ateral patellar tilt and minimal arthrosis. This article discusses dia
gnosis of anterior knee pain, nonoperative treatment, and relative adv
antages of open and arthroscopic lateral release.
