From February 1987 till March 1989 131 patients were operated for vert
ebrobasilar insufficiency caused by anomalous retrograde origin of the
A. vertebralis. 131 operations were performed: 59 reinplantations of
the A. vertebralis in the A. subclavia, 79 rotations and fixations of
the A. subclavia to the first rib. No deaths or strokes occured during
the early postoperative period, A recurrence of neurologic symptoms w
as observed in 54 patients (52.9 +/- 2.4%) during the mean follow-up p
eriod of 59.4 +/- 1.4 months. We conclude that anomalous retrograde or
gin of the A. vertebralis from the A. subclavia is an indication for o
peration, if the role of this pathology for genesis of vertebrobasilar
insufficiency has been proven. The monophotone emission tomography of
the brain is an important method of diagnosis. The operation of choic
e is the reimplantation of the A. vertebralis in the A. subclavia.
