A case of a long-standing lesion on the hand of a 64-year-old man is r
eported. Histology of the excision specimen showed characteristic feat
ures of mucinous syringometaplasia. The histopathological findings are
described in detail including the first immunohistochemical study of
this entity. Previous cases in the literature are reviewed. Mucinous s
yringometaplasia is a benign, presumably reactive verrucous lesion mos
t commonly occurring on acral sites. Complete excision is curative.
