A case of severe retrograde and less severe anterograde amnesia with i
nitial multimodal agnosia is described as an outcome of traumatic brai
n damage. Anatomical and metabolic studies document right frontal and
left occipital lesions with sparing of medial temporal cortex. The cas
e demonstrates the common basis of memory and perception, and is inter
preted as a result of disruption to a semantic phase in the process of
object formation.
