Secondary hyperparathyroidism is seen in most cases of chronic renal f
ailure, yet rarely requires operative treatment. The pathophysiology i
s complex, involving the kidney, liver, intestine, and bone in additio
n to the parathyroid gland. Although medical management suffices for m
ost cases, the remainder require parathyroidectomy. The indications fo
r operation and extent of treatment are discussed.
