Abstract
Clinical factors associated with urinary albumin excretion (UAE) in ty pe II diabetes are less well known than in type I diabetes. To examine the factors associated with UAE in type II diabetes, 933 Appropriate Blood Pressure Control in Diabetes Trial patients were classified acco rding to UAE status: normoalbuminuria (<20 mu g/min), microalbuminuria (20 to 200 mu g/min), and macroalbuminuria (>200 mu g/min). The class of UAE was then correlated with various clinical factors. Using univa riate analyses, Hispanic ethnicity, African-American race, male gender , poor glycemic control, insulin use, long duration of diabetes, dysli pidemia, diastolic and systolic hypertension, smoking, and obesity wer e significantly correlated with microalbuminuria and macroalbuminuria. Using multivariate logistic regression analyses controlling for diabe tes duration, glycosylated hemoglobin, gender, and race, the most sign ificant predictors of microalbuminuria and macroalbuminuria were systo lic hypertension, body mass index, high-density lipoprotein cholestero l, insulin use, and smoking pack-years. Of these factors, several are potentially reversible with aggressive intervention. (C) 1995 by the N ational Kidney Foundation, Inc.
