Major depression with psychotic features (PMD) represents a distinct a
nd severe subtype of depression. This article reviews characteristics
that distinguish PMD from nonpsychotic major depression. Emphasis is p
laced on increased activity of the hypothalamic-pituitary-adrenal axis
and dopamine systems in PMD. Interactions between the two systems are
reviewed as a means for understanding the biological basis for psycho
tic thinking in depressed patients. Strategies for addressing key rese
arch questions in this area are discussed.
