In a patient with respiratory distress and congestive heart failure at
the age of one month, echocardiography revealed the anomalous origin
of the right pulmonary artery from the ascending aorta. Diagnosis was
confirmed by angiocardiography, and corrective surgery was performed.
The diagnostic approach by echocardiography, angiocardiography and MR
imaging is described. The anomaly leads to complex hemodynamic changes
with left-ventricular volume overload and reflectory increase of pulm
onary resistance which requires corrective surgery. The procedure of c
hoice with re-implantation of the right pulmonary artery is explained.
