We describe a rare case in whom tension pneumocephalus (TP) occurred a
fter transsphenoidal resection of a pituitary tumor. TP led to increas
ed intracranial pressure and mental changes following the surgery. Man
agement consisted of occlusion of lumbar drainage, placing the patient
in a flat or slight Trendelenburg position, and oxygen administration
followed by surgical repair of the leak at the base of the skull.
