Abstract
Assessment of the effects of closure of exomphalos on ventilatory mech anics is pivotal to primary management decisions. We present a patient in whom problems of abdomino-visceral disproportion were compounded b y reduced thoracic volume due to Poland's syndrome. Initial nonoperati ve management with delayed closure of the ventral hernia using epidura l anaesthesia was successful.
