In a 17-day-old newborn with systemic candidiasis fluconazole proved i
neffective to prevent the progression of the disease. Fluconazole had
been recommended as a treatment alternative to amphotericin B in syste
mic candida infections. In our case, progression of the infection and
a threatening lung failure made necessary a switch to amphotericin-B a
nd 5-flucytosine. Despite high serum trough-levels and fluconazole-sen
sitivity of the candida albicans species, the infection could not be c
ontrolled. In this case report the efficacy of fluconazole in dissemin
ated candida infection is discussed.
