Abstract
We describe the anaesthetic management of an elderly woman who underwe nt thoracoscopically assisted trans-hiatal oesophagectomy One-lung ven tilation with insufflation of carbon dioxide was necessary for adequat e surgical access. Potential intra-operative problems included hypoxae mia during one-lung ventilation, mediastinal shift and inadvertent dam age to mediastinal structures.
