25 patients with a Kock Pouch were reevaluated 366 days after surgery.
Early complications occurred in 42 %, late complications were found i
n 24 %, including pulmonary embolism, nipple necrosis, incomplere nipp
le slipping and stone formation inside the pouch. 12 % of our patients
required operative revision of the nipple valves. 88 % proved to be s
ocially continent. Urodynamic studies classified 96 % of the pouches a
s low-pressure reservoirs. Renal function was preserved, neither elect
rolyte imbalances nor chronic manifest metabolic acidosis occurred. Vi
tamin B-12-resorption was reduced in 85 %, while serum levels decrease
d in 4 patients. Histologically, a reduction of the resorptive surface
, signs of chronic inflammation and a constant number of globet cells
were found. With a low morbidity and reoperation rate, the Kock Pouch
fulfills the demands of a modern urinary diversion.
