A 63-year-old man presented with pulmonary infiltrates and maxillary s
inusal opacities. Although necrotizing vasculitis and granulomatous in
flammation were present in nasal mucosa, pulmonary biopsy revealed ade
nocarcinoma. Simultaneous appearance of granulomatosis in the maxillar
y sinus and necrotic lung nodule does not necessarily mean that the lu
ng abnormalities are also granuloma. This case report suggests that di
fferential diagnosis between Wegener's granulomatosis and lung carcino
ma may be difficult.
