Abstract
There are multiple etiologies reported as causes of lung abscess; howe ver, this differential rarely includes intra-abdominal abnormalities o ther than extension of a hepatic process. We describe a patient who wa s found to have a lung abscess and empyema resulting from the developm ent of a nephrobronchial fistula secondary to nephrolithiasis and pyel onephritis. The patient had no urinary symptoms or known abdominopelvi c infection and the etiology of lung abscess was only incidentally dis covered after chest CT revealed extension of pleural fluid below the d iaphragm.
