Adrenal histoplasmosis is a rare infection that can be misdiagnosed as
tuberculosis. We present here a case of adrenal histoplasmosis in a 6
5 year-old male diabetic with marked weight loss. Laboratory investiga
tions noticed an inflammatory syndrome and the abdominal computed tomo
graphy scanner reported an heterogenous left adrenal mass of 6 cm in d
iameter. Hormonal as well as bacteriological studies were negative. Th
e patient was operated and the histopathological examination proved th
at the mass was a tuberculoma and an anti-tuberculous treatment was st
arted. Four months later, the patient suffered from recurrence of symp
toms and laboratory investigations confirmed the inflammatory syndrome
and the abdominal syndrome and the abdominal computed tomography scan
ner showed a right adrenal mass. A surgical biopsy was performed and s
pecific fungal researches proved that the lesion ws due to Histoplasma
capsulatum. The patient experienced a remarkable improvement under an
ti-fungal treatment.
