Photopheresis is being used with increasing frequency as therapy for p
atients with neoplastic and dermatologic diseases and is being evaluat
ed as therapy for patients with AIDS. We describe a patient with advan
ced cutaneous T cell lymphoma who developed pulmonary and cutaneous cr
yptococcosis after receiving therapy with photopheresis and biweekly m
ethotrexate. We consider the potential roles of cutaneous T cell lymph
oma, methotrexate, and photopheresis as predisposing factors in the de
velopment of serious cryptococcal infections.
