The course of renal osteodystrophy possibly includes changes, which ca
n mistakenly considered malignant. Especially dialysis-associated tumo
ral calcinosis may mislead to an inadequate aggressiv surgical procedu
re. Spontanous involution of these lesions takes place after subtotal
parathyreoidektomy. Brown tumors subsequent secondary hyperparathyreoi
dism are extremely rare. During maintenance hemodialysis frequently ap
pearing intraosseous cysts develop secondary to beta-2-microglobulin-a
myloidosis. Surgery only should be performed in case of occuring patho
logic fractures. Other cases are treated conservatively under continou
s nephrologic supervision.
