Hypersexuality and paraphilic behavior have been associated with a var
iety of focal brain lesions but have rarely been documented in multipl
e sclerosis (MS). We describe a woman with neuropathologically confirm
ed MS who developed multiple paraphilias, disinhibition, and hypersexu
ality prior to her death. The patient had extensive lesions involving
basal frontal, perithalamic, septal, hypothalamic and temporal regions
. Lesions that involve the frontal lobes and diencephalic structures i
nfluence sexual behavior.
