A 28-year-old renal transplant patient developed a shock due to an acu
te bleeding from his right native kidney. An urgent nephrectomy was pe
rformed. Macroscopic and histological examination revealed a pyelocali
ceal diverticulum as the cause of the hemorrhage. This rare event must
be taken into account in every case of an unexplained acute renal ble
eding.
