We report a case of interstitial lung disease developing during treatm
ent with chlorambucil, vinblastine, procarbazine and prednisone for ex
trapulmonary Hodgkin's disease. Transbronchial biopsies demonstrated n
oncaseating epithelioid cell granulomata, and bronchoalveolar lavage f
luid showed an increased percentage of lymphocytes, with a raised T ce
ll helper-suppressor ratio, suggesting a diagnosis of sarcoidosis. Tre
atment with corticosteroids brought about complete resolution of the r
adiographic lesions. A possible relationship between sarcoidosis and l
ymphoma is discussed, and a short review of the literature is given. T
his case report has some unique features that, to our knowledge, have
not been published before.
