A case of posterior interosseous nerve paralysis is reported with disc
ussion of some characteristics that appear to distinguish it from entr
apment neuropathy and neuralgic amyotrophy. The surgical implications
are also discussed. More than ten similar cases have been reported, bu
t the pathogenesis of this condition is still controversial. The patie
nt presented with posterior interosseous nerve paralysis related to fo
cal radial nerve constriction secondary to vasculitis in the perineuri
um. The constriction site was resected and the radial nerve was suture
d. The patient recovered completely after 8 months.
