We describe a phacoemulsification technique that uses an anterior cham
ber maintainer (ACM) to control intraocular pressure. Continuous irrig
ation through the ACM maintains pressurized intraocular conditions thr
oughout surgery and when the automated system is not activated. Intrao
cular pressure is stabilized at predetermined levels with minor fluctu
ations. Viscoelastics can be used in conjunction with the ACM.
