In an 81 year old bedbound male admitted to a dermatology ward for pre
ssure sores over both heels, a severe spinal deformity with-diffuse jo
int ankylosis and a bluish discoloration of the sclerae and skin over
the face suggested alcaptonuric ochronosis. The diagnostic criteria fo
r this rheumatic disease are reviewed. The differential diagnosis with
ankylosing spondylitis is discussed.
