Three cases of pseudocysts or hemorrhagic cysts of the adrenal gland w
ere encountered in female patients who presented with loin pain. The a
drenalectomy specimens contained cystic structures; 2 had fibrous wall
s containing organizing blood clot, the remaining lesion was thin wall
ed and contained blood stained fluid. Fat, normal adrenal cortical tis
sue and calcification were found within the cysts. These lesions (aris
ing from vascular anomalies) require separation from hemorrhagic adren
al neoplasms. Awareness of adrenal pseudocysts and careful attention t
o the histological features aids this distinction.
