Collagenous colitis is an uncommon cause of chronic watery diarrhea, c
haracterized by colonic deposition of collagen. Nonerosive, oligoartic
ular, peripheral arthritis has previously been noted in about 7% of pa
tients with collagenous colitis. We describe a patient with collagenou
s colitis who concurrently developed erosive, seronegative spondyloart
hropathy affecting peripheral and axial joints. Synovial histology was
characterized by a conspicuous inflammatory infiltrate comprised of h
istiocytes, lymphocytes and plasma cells. Collagenous colitis is sugge
sted to be a systemic autoimmune disorder, with extraintestinal featur
es such as thyroiditis and arthritis.
