A 2.5-year-old patient who presented with abdominal pain, melena, feve
r, and subileus underwent an exploratory laparotomy. Apart from adhesi
ons, edema, and friable tissue in the right pararectal space together
with some 15 ml black fluid coming from the right pararectal and extra
peritoneal portions of the pelvis, no pathology was found. A few days
later, the diagnosis of a rectal duplication was arrived at after ultr
asound and CT scans of the pelvis together with rectoscopy and filling
of a pararectal cavity by contrast material. The duplication was exci
sed by the perineal approach; it was covered by gastric mucosa, with s
igns of perforation at its superior pole.
