Follicular mycosis fungoides is a rare variant of cutaneous T-cell lym
phoma. We report a patient with thin plaques of mycosis fungoides stud
ded with follicular papules. Histologically, these lesions were compos
ed of dense infiltrates of lymphocytes within and around follicular ep
ithelium (''folliculotropism''). This case indicates that follicular m
ycosis fungoides is a distinct clinicopathologic entity. It is differe
nt from follicular mucinosis associated with mycosis fungoides. Concep
tually, the relationship between follicular mycosis fungoides and ordi
nary mycosis fungoides is analogous to the relationship of lichen plan
opilaris to ordinary lichen planus.
