Three cases of plant thorn synovitis of the knee responsible for recur
rent monoarthritis are reported. Diagnosis of this rare cause of monoa
rthritis is often delayed. Demonstration in a histological specimen of
a foreign body reaction surrounding a thorn is the only means of esta
blishing the diagnosis. Removal of the foreign body is essential and u
sually requires complete synovectomy. Computed tomography or magnetic
resonance imaging can show the thorn, allowing more limited surgery.
