A clinical-histologic and morphometric study on 32 patients with preca
ncerous laryngeal lesions was carried out. Two biopsies taken at diffe
rent times-were studied, and the cases in which carcinoma developed we
re compared with those cases in which it did not. The only clinical-hi
stologic variable with prognostic value, related with evolution to car
cinoma, was severe dysplasia. The Malignancy Grading System, which gro
uped together 13 clinical-histologic variables, had both high sensitiv
ity and high specificity. On the other hand, quantitative microscopy w
as a more sensitive and,objective method. Through the relationship bet
ween the parameters of orientation/nuclearity and Histometric Progress
ion Index, we could separate those cases in which invasive carcinoma-d
eveloped from those cases in which ii did not with a sensitivity of 10
0% and a specificity of 82%.
