We report the case of an Arias-Stella reaction of the cervix presentin
g in the cervical smear from a 35-year-old woman who complained of hea
vy menstrual periods. The smear was originally reported as inconclusiv
e. Colposcopy revealed an abnormal transformation zone extending out o
f range. cervical conization was performed. Histologic examination rev
ealed an ectopic cervical pregnancy associated with a cervical Arias-S
tella reaction. The cytologic manifestations of the Arias-Stella react
ion continue to cause diagnostic confusion with benign lesions and ade
nocarcinoma. We believe this to be the first reported case of a cervic
al Arias-Stella reaction associated with a cervical pregnancy; it shou
ld alert cytologists to the diagnostic implications.
