A total of 40 patients with Functional Class III-IV stable angina pect
oris were examined. Silent myocardial ischemia was detected in 87.5% o
f patients by using 24-hour Holter monitoring. A statistically signifi
cantly greater amplitude and longer duration of pain episodes were obs
erved. The patients underwent exercise echocardiography. Functional ex
ercise was performed by frequent-increasing transesophageal cardiac pa
cing. The index of local myocardial contractility disturbance was asse
ssed and calculated as the product sum of the number of diseases segme
nts by the rating coefficient: hypokinesia, 1 score, akinesia, 2 score
s, and dyskinesia, 3 scores. There was a statistically higher segmenta
l asynchronism in patients having a silent myocardial ischemic episode
of more than 15 min. The index of local contractility disturbance in
this group of patients was 7.9 +/- 3.2 units. There was a relationship
between the duration of silent myocardial ischemia and the presence o
f heart failure. It was concluded that the duration of silent myocardi
al ischemia might be used as an additional criteria for assessing the
severity, prognosis and adequancy of therapy in patients with stable a
ngina on effort.
