Abstract
This report describes surgical treatment consisting of mitral valve re pair and septal myectomy in a young patient with a combination of hype rtrophic cardiomyopathy and intrinsic mitral valve disease. The poster ior mitral valve leaflet was elongated and was subject to systolic ant erior motion, creating left ventricular outflow tract obstruction. Wit h the described treatment, it was possible to avoid mitral valve repla cement in this patient.
