Acute complete oculomotor palsy with headache is a classical presentat
ion of an extrinsic compression most commonly due to a posterior commu
nicating artery aneurysm. We present a patient with such a presentatio
n but with histologically proven giant cell arteritis. This possibilit
y should be considered especially in the elderly to avoid complication
s and the need for angiography.
