A patient with pseudotruncus arteriosus who presented with a large med
iastinal mass due to a systemic-to-pulmonary collateral artery aneurys
m is reported. This aneurysm caused tracheal compression with resultin
g dyspnea and postobstructive pneumonitis, which are unusual presentin
g features in these patients. The differential diagnosis for a mediast
inal mass arising in a patient with pseudotruncus arteriosus, or any o
ther patient with possible systemic-to-pulmonary collateral arteries,
should include aneurysm.
