The authors report two cases of cystic adventitial disease of the popl
iteal artery. Both patients complained of calf claudication and presen
ted a thrombosis of the popliteal artery. In one case, magnetic resona
nce imaging exhibited a specific preoperative diagnosis. In both cases
, at least one communication between the cysts and the capsule of the
knee joint was demonstrated, supporting the congenital hypothesis of t
he disease. The two cases have been treated by vein graft replacement
of the occluded popliteal arteries. Both patients have remained asympt
omatic and the two grafts have remained patent during the ninth and tw
elfth month follow-up periods.
