We report on the accidental division of the ureter of a transplanted k
idney during laparoscopic marsupialization of a symptomatic lymphocele
into the peritoneal cavity. Identification of the lymphocele was diff
icult because of a thick layer of overlying periperitoneal fibrolipoma
tous tissue. The complication was identified intraoperatively and sten
ted reanastomosis of the divided ureter was performed. Postoperative c
ourse was uneventful. To avoid accidental injury to the transplanted u
reter during laparoscopic drainage of lymphocele, we recommend that a
ureteral catheter be inserted.
