A case of a cadaveric kidney transplant recipient who developed progre
ssively severe renal failure within 3 mo of transplantation secondary
to renal artery stenosis is presented. The patient was primarily hypot
ensive and Doppler ultrasound showed normal flow. The problems in diag
nosing this unusual case are reviewed. The findings on serial radionuc
lide studies eventually led to consideration of the correct diagnosis.
