The diagnosis of wide complex tachycardia based on surface electrocard
iography can be difficult. Misdiagnosis occurs frequently and is commo
nly associated with increased morbidity and mortality. We describe a c
ase of wide ORS complex tachycardia in which transesophageal electroca
rdiography and intravenous adenosine were used to obtain a reliable di
agnosis. These are safe and readily available tools for elucidating th
e mechanism of wide complex tachyarrhythmias in hemodynamically stable
patients.
